Citrate anticoagulation for continuous renal replacement therapy
نویسنده
چکیده
Clinical Problem A 73 year old female patient presented to the Accident and Emergency Department (A&E) with a profound anaemia, acute kidney injury (AKI) and severe hyperkalaemia. She had a background of Chronic Obstructive Pulmonary Disease (COPD), osteoarthritis, hypertension, morbid obesity and a very poor functional status. She described a one week history of bleeding haemorrhoids. She was not actively bleeding at the time of presentation.

Role of citrate and other methods of anticoagulation in patients with severe liver failure requiring continuous renal replacement therapy
Anticoagulation is required during continuous renal replacement therapy to prevent filter clotting and optimize filter performance. However, anticoagulation may also be associated with serious bleeding complications. Patients with liver failure often suffer from underlying coagulopathy and are especially prone to anticoagulation complications. The aim of this review is to present the unique fea...
متن کاملCitrate anticoagulation for continuous renal replacement therapy (CRRT) in patients with acute kidney injury admitted to the intensive care unit
Continuous forms of renal replacement therapy (CRRT) have become established as the treatment of choice for supporting critically ill patients with acute kidney injury. Typically, these patients have activation of the coagulation cascades, peripheral mononuclear cells and platelets, but also a reduction in natural anticoagulants, and are therefore prothrombotic. For continuous modes of renal re...
متن کاملBlood transfusion requirements and filter longevity with regional citrate anticoagulation compared with heparin
Introduction Citrate is the recommended anticoagulant for continuous renal replacement therapy (RRT) [1], and is thought to confer numerous benefits, including more continuous hours of filtration, fewer total circuits used, less overall cost and maybe improved patient and kidney survival when compared with heparin anticoagulation [2]. Our ICU changed from heparin to citrate anticoagulation in J...
متن کاملRisks and benefits of citrate anticoagulation for continuous renal replacement therapy.
Heparin, despite its significant side-effects, is the most commonly used anticoagulant for continuous renal replacement therapy in critical care setting. In recent years, citrate has gained much popularity by improving continuous renal replacement therapy circuit survival and decreasing blood transfusion requirements. However, its complex metabolic consequences warrant modification in the desig...
متن کاملEvaluating the safety and efficacy of regional citrate compared to systemic heparin as anticoagulation for continuous renal replacement therapy in critically ill patients: A service evaluation following a change in practice.
Following the implementation of citrate anticoagulation for continuous renal replacement therapy, we evaluate its first year of use and compare it to the previously used heparin, to assess whether our patients benefit from the recently reported advantages of citrate. We retrospectively analysed 2 years of data to compare the safety and efficacy of citrate versus heparin. The results have shown ...
متن کاملComplications of regional citrate anticoagulation: accumulation or overload?
Regional citrate anticoagulation (RCA) is now recommended over systemic heparin for continuous renal replacement therapy in patients without contraindications. Its use is likely to increase throughout the world. However, in the absence of citrate blood level monitoring, the diagnosis of citrate accumulation, the most feared complication of RCA, remains relatively complex. It is therefore common...
